Provisionally Registered Pharmacist (PRP) Notes in Outpatient Department at Specialist Hospital

Today I would like to share some notes related to pharmacy during PRP training at the outpatient (OPD). Hope this will help some of the readers who would like to get some picture of how is the learning during PRP time. However, your situation may vary depends on the location and people.

The notes below are the important point that is being highlighted during briefing.

(1) CALCULATION OF RATIO STRENGTH. 
The picture below is captured from a pharmacy textbook for calculation. It is important to know how to convert percentage to ratio and vice versa. 
(2) Examples of intervention done.
These are the old type of logbook. This will train you to become alert and know which dose /situation that need to intervene. It is important to know the correct dose/medication regime/etc before contacting prescriber to discuss/advise /recommend changes. However if the prescriber is unreachable, write an intervention note and give back to the patient. It is important that patient get the correct dose and correct medication.

(4) Page 2. Flow in OPD. List of counselling devices.
(5) Page 3. Counselling for hydrocortisone enema, nasal spray, nasal douching, nystatin syrup.
(6) Page 4. Counseling for insulin.
(7) Page 5. Continue from insulin counseling.
(8) Page 6. Continue from insulin counseling.
(9) Page 7. Counselling respiratory devices.
(10) Page 8. Turbuhaler, Accuhaler, Handihaler, Breezhaler.
(11) Page 9. Respimat, Easyhaler, Warfarin,
(12) Page 10. Continue warfarin. Side effect counselling.
(13) Page 11. Methadone.
(14) Page 12. Topical /Skin application.
(15) Exclusion criteria (1) for foreigner that want to collect medication.
(16) Exclusion criteria (2) for foreigner that want to collect medication.
(17) Definition of haematinics. The dose depends on the practice of each unit.
(18) Vitamin D physiology
